Consent Forms

All consent forms must be completed and received by the UBC Camps office before the first day of camp unless otherwise stated below.

UBC Camps

This form is for all UBC Camps except for Aquatics Play & Swim, Sailing & all Windsure camp participants. The appropriate consent forms for these programs can be found below.

NOTE: If you have registered by faxing in a Registration Form you do not need to submit a consent form.

MacSailing Camp

This form must be completed for all MacSailing camp participants. Please bring the form to MacSailing on the first day of camp.

UBC Ropes Camp

This form must be completed for all UBC Ropes camp participants. Please bring the form to UBC Ropes on the first day of camp.

Windsurfing and Skimboarding Camps

This form must be completed for all Windsurfing and Skimboarding participants. Please bring the form to Windsure on the first day of camp.
